How frequently?

Weekly, fortnightly etc

What time?

The more specific the better! If you know you're going to be meeting at a specific time that's great.

Do you have a start (and end) date?

For example if you're wanting to run your group termly.

What's the maximum number of people who can join?

A brief desciption of what your group would involve

It would be useful to include how your group will have an UP (connecting with God), IN (caring for those in the group) and OUT (reaching those who don't know God). Different groups will have different emphases which isn't a problem.
